Is your past or current Creatingsmiles Orthodontic patient a senior in high school? The CreatingSmiles College scholarship aims to support the educational goals of a deserving high school senior pursuing a career in a science or math field. This scholarship will be awarded based on the applicant’s academic achievement and interest in furthering their academic career. The $1500 scholarship is given to a deserving applicant. We reserve the right to split the scholarship should more than one excellent candidate apply and not give the scholarship should there be a lack of in our opinion qualified applicants.

If you are a past CreatingSmiles Orthodontic patient and a local student with a passion for science, we encourage you to apply for this scholarship. To submit your application, please forward the following printed and signed forms below to our office by December 1st. Email submissions will not be considered. Be sure to attach all documents and answer the scholarship question. We look forward to receiving your application.
